You can view openings in the schedule and select the time that is most convenient for you.

Either way we can fit you into the schedule, and get you on your way to health!

Here are some IMPORTANT things to know:

You can download and complete the  New Patient Packet here. 

**Please arrive at least 20 minutes prior to your appointment time to complete our new patient forms!  Unfortunately, if you arrive late without the forms completed, we'll need to reschedule you. This is because we strive to stay on time and not keep people waiting!

Our paperwork is very detailed and extensive - it allows us to gain a comprehensive insight into your health.  Please complete as completely as possible, well before your appointment!


New Patient Deposit Required

We require a $100.00 deposit for holding new patient appointment time slots. This fee is NON-refundable if you do not keep your new patient appointment (for any reason). You are asking us to reserve one hour and 15 minutes out of our very busy schedule. If you cancel or miss your new patient appointment, this fee is NON-refundable. 

When you do keep your appointment time, this fee is applied to you first-day visit charges. Note - all credit card info is kept very safe and confidential.

A member of our staff will contact you to collect your deposit within 48 hours of scheduling; you may also call us with your card information as well.

Driving Directions

We are in mid-town Fort Collins - Main cross streets are Drake and Shields - We're on Shields and Centre Ave.

From North Fort Collins & Wyoming:

  • Head South (toward Loveland) on College.
  • Turn right (west, toward the mountains) at Drake Road.
  • Go to Shields Street and take a Right. 
  • Make a right onto Centre Avenue and we're on the left in the Centre 
           Professional Office Park. 
  • Look for large maroon lettering on the building that says 1030-B


    • From Windsor, Greeley and Denver:

  • From I-25, exit onto Harmony Road.
  • Take Harmony to College
  • Go Left on Drake Road
  • Follow the above directions.


    • From Loveland, Longmont and Boulder via Highway 287:

  • Once Highway 287 (College Avenue) intersects with Drake Road, turn LEFT (toward the mountains) onto Drake.
  • Go to Shields Street and take a Right. 
  • Make a right onto Centre Avenue and we're on the left in the Centre  
           Professional Office Park. 
  • Look for large maroon lettering on the building that says 1030-B
